Please bring a copy of any tests which have been performed on you in the last 1-2 years. If these tests consist of xrays, MRI, or CAT scan please bring the film as well as the report to our office on the initial visit. I will need this information before the start of treatment. If you can not obtain this information from the doctor who ordered the test prior to the initial visit, you will be asked to fill out a records release form so that our office may obtain this information. If the tests were taken at the hospital, you will have to obtain these records in person.
Patients will be required to fill out patient history forms, insurance forms, and HIPPA forms on the initial visit.
A case history will be developed based on the patient information forms and questions asked to the patient prior to the examination of the patient.
An initial examination will include an evaluation of the area involved and any appropriate neurologic/orthopaedic tests.
Plain film xrays of the involved area will be taken to confirm exam findings, determine the presence or absence of conditions such as scoliosis and arthritis, and any congenital anomalies which would influence the type of adjustment and therapy a patient receives.
A review of exam and xray findings with the patient will occur on the second visit. The doctor's treatment plan and recommendations will be included with the review.
